Q: Why do I have to sign an Assignment of Benefits/Appointment of Authorized Representative form?
A: This allows Precision to file the claim to your health plan on your behalf and to pursue an appeal in the event your health plans fails to provide coverage or adequate reimbursement for the ChemoFx® test.
Q: Who pays for the test
A: The ChemoFx Assay is covered by Medicare Part B and has been reimbursed by over 400 commercial insurance companies. Precision will provide you with information that may help you determine the likelihood of your plan providing payment.
Precision will accept assignment from your commercial health plan provided that you complete the Assignment of Benefits/Appointment of Authorized Representative form. Additionally, Precision will submit claims on your behalf to your insurance company. Patients with commercial insurance will be responsible for applicable co-payments, deductibles and co-insurance as required by their policies.
Q: What happens if my insurance company does not pay at all for ChemoFx?
A: Precision bills Medicare directly and does not bill Medicare beneficiaries or Medicare HMO /PPO plan members.
If Precision has a completed Assignment of Benefits/Appointment of Authorized Representative form signed by you, Precision will aggressively pursue payment from your insurance company.
In the event you receive a letter, statement, or an Explanation of Benefits from your insurance company prior to Precision contacting you, it is essential that you contact a Precision billing specialist before you contact your insurance company. Contact Precision as soon as possible in order to prevent unnecessary coverage and/or reimbursement delays.
