Q: Who pays for the ChemoFx® test?
A: The ChemoFx Assay is covered by Medicare Part B and has been reimbursed by over 400 commercial insurance companies.

Precision will accept assignment from the patient's commercial health plan (i.e. Precision Therapeutics will not balance bill the patient) provided that the patient completes the Assignment of Benefits/Appointment of Authorized Representative form. Additionally, Precision Therapeutics will submit claims on behalf of the patient to the patient's health plan. Patients with commercial insurance will be responsible for applicable co-payments, deductibles, and co-insurance as required by their policy.

Q: If the insurance company does not pay at all for the ChemoFx® test, will Precision Therapeutics bill the patient?
A: Precision Therapeutics bills Medicare directly and does not bill Medicare beneficiaries or Medicare HMO /PPO plan members. If Medicare does not reimburse, Precision will pursue an appeal of the denial

If Precision Therapeutics has a completed Assignment of Benefits/Appointment of Authorized Representative form signed by the patient, Precision Therapeutics will aggressively pursue payment from the patient's insurance company. Patients will be responsible for co-payments, co-insurance, and deductibles as required by their policy. Patients may be eligible for compassionate care depending on their individual situation (please refer your patient to a Precision Therapeutics billing specialist for further details).

Q: How do I begin submitting specimens to Precision Therapeutics? A: In order to get started, you may contact a Precision Therapeutics account specialist to assist you with any questions or concerns. You may contact a Precision account specialist at 1.800.547.6165.

Q: If the patient's insurance company calls me, do I need to get involved? A: Because ChemoFx® is a highly specialized test, insurance companies may call you to affirm the medical necessity of the test. Precision may ask you to provide a letter of medical necessity on occasion.

If questions regarding billing issues arise, please direct these callers to the Precision Therapeutics Billing department at 1.866.243.6639. Precision's billing staff is well qualified and prepared to assist you. It is Precision's desire to minimize any burden that arises regarding reimbursement issues.

Q: If the patient calls me directly, do I need to get involved? A: If a patient has any billing issues whatsoever, the patient should contact Precision's billing department at 888- 522-7087.
